The first permanent molar, often referred to as the “pillar of occlusion,” plays a crucial role in the overall dental arch’s integrity and function. However, this tooth is frequently subjected to significant destructive forces due to caries, trauma, and periodontal disease, leading to clinical scenarios where interceptive extraction becomes a necessary therapeutic intervention.

In this presentation, we will explore the latest methodologies and techniques for the interceptive extraction of badly destructed first permanent molars. Emphasizing a multidisciplinary approach, we will review pre-operative assessments, including radiographic evaluations and the identification of critical anatomical considerations to minimize complications. We will discuss the indications for interceptive extraction, highlight the importance of proper extraction timing, and the long-term follow-up to ensure optimal functional and aesthetic outcomes.
